First a little about me.. My name is Meredith and my journey with Aged Care started when I applied to be a volunteer helping out at my local nursing home. While I was being interviewed for this role I was offered a job as a Personal Carer. It was then I left the job I was in and stepped way out of my comfort zone. I started as a Personal Carer then completed my Enrolled Nursing and six years after that I completed my Bachelor of nursing. During this time I have experienced ward nursing, aged care and practice nursing. My practice nursing has been mainly Chronic Disease Management which I feel has led me to 'Believe u can Care'.

Combining my previous work with aged care and the skills I have learnt as a practice nurse I have now entered a new phase of assisting GP's with their health assessments in nursing homes. I feel this model of care will benefit practices to have an independent party assisting with the annual comprehensive medical assessment. I can also assist with working out reviews ect which maximise patient care as well as billing opportunities.Please contact me if you are interested in more information.
